General Forms

• Membership Application. Use this form to apply for membership in IFSHA for owners, competitors, or breeders.
• Horse Membership Application. Use this form to register your horses with IFSHA.
• Equine Identity Record. Use this form to report the features of your horse that IFSHA and show staff can use to positively indentify your horse. Possible identification methods are electronic id chips (RFID), brands, and tattoos.
